The Citizenship Form is now required 14 days in advance of a cruise and most foreign itineraries. If you are planning to travel with us, please submit the form below or print and fax it to us at (309) 266-8384.

For passengers traveling to an area that requires a passport or certified birth certificate, this form should be submitted a minimum of 14 days before your expected departure date. Name/s must be as indicated on your passport or certified birth certificate. All passengers (including children) must have the proper identification papers, and it is the responsibility of the traveler to determine which documents (passport, certified birth certificate, and/or visa) are needed for the travel itinerary.
